NSF Reporting Requirements
WiscAMP is required to report to the NSF on all activities and persons related to funded projects, activities, and programs. Please use the forms below when reporting to WiscAMP the outcomes of a Small Grant or WiscAMP-sponsored project or activity.
Activity Worksheets
- Form-1 Summary of Grant Project (required of all WiscAMP funded projects)
- Form-2 Activity Sheet (required if project involves more than one activity)
- Form-3 Student Demographic Sheet (required information from all students directly funded through WiscAMP)
- Form-4 Faculty/Coordinator Demographic Information (required information from all faculty or staff involved with the project)
- Form-5 Student Sign-In Sheet (required information from funded and non-funded students participating in specific activities)
- Form-6 Faculty Sign-In Sheet (required information from all faculty or staff participating in specific activites)
